First Visit

Your first visit can be divided into two parts, which are the intake and consultation and/or treatment.

The intake appointment consists of filling out paperwork. This includes health, personal and billing information. To make this process easier and more efficient, please remember to bring a complete list of any medications you are currently using. For patients using dental insurance, please have all necessary information available at intake. This information includes name of insurance carrier, group name and number as well as social security number and date of birth for the primary insured and subscriber.

The second part of your visit will be the consultation and/or treatment. Most cases will start with radiographs and an examination. After this is done, we will discuss the findings and all the options. The final decision on which treatment will or will not be done is entirely up to you. Please keep in mind you may be required to return for more visits depending upon the findings of your examination.
